This form underlines the agreement between social-dance.ca representative and any individual(s) that agree(s) 

to purchase dance lesson(s). The purchasing individual(s) must oblige to the following dance lesson details as well as the terms and conditions pertaining to each dance lesson.    

Dance lesson Details: 

Lesson duration: 60 minutes (55 minutes of dance instructions + 5 minute break)

Wedding dance lesson duration: 45 minutes

Terms and Conditions:

1. Lesson Fees: All lesson(s) fees must be paid in full prior to the start of each lesson.

2. Lesson Expiration: Individual MUST take at least one Private Lesson per week upon signing this  agreement.   

     Example: 4 lessons purchased will expire within 4 weeks of the agreement signed date. However, this timeline can   

     be altered by individuals who wish to reschedule(see Reschedule Policy below).

3. Refund Policy: social-dance.ca does not provide refunds on any missed or expired lesson(s).

4. Reschedules and Cancellations Policy: Private lesson(s) should be rescheduled or cancelled 7 days prior to the date of   

      the lesson for a full refund.

5. Waiver and Liability policy: social-dance.ca management and representatives will NOT be reliable for any personal 

      injury or property damage arising out of or connected to participation  in any and all Dance Lesson Activities, use of Equipment, 

      or any and all acts or omissions, including negligence by representative(s). This waiver and release of liability also includes, but is      not limited to, damage to property, Equipment and the premises caused by any individual during the lesson.

6. Photography: social-dance.ca is authorized to use, any photos taken of individuals during lessons, events and/or 

      performances, at their discretion for promotional interest to the social-dance.ca

 7. This agreement remains valid, with no expiration, as long as the individual continues with his/her dance 

       lessons. 

I have read and understood the terms and conditions of this agreement, before I buy any dance lesson

(private dance class, monthly group classes or any package) from social-dance.ca
